Description

A home care assessment is used to identify the current and future patient care needs. A case plan can then be made to support the area/s of need. Some of the information on there will be getting to know your likes/dislikes, what your daily routine consists of, etc.

Cost for Services

There are some insurances that will pay for your care and the insurance that FFH can take is Medicaid. Payments will vary in cost due to the fact that each service plan, for an individual, is specially modified or designed for their needs. This is something that we would go over in our initial meeting.

More Information

Each care plan is custom made and so no two plans are alike. With that said, cost will vary. There is private pay and some may be covered by insurance you have or by Medicare or Medicaid.    

Facility Care Explained

Yes, we can provide services in facilities if extra assistance is needed. Whether it is your own home, nursing facility, or someone that is living with a friend or family member. We go wherever it is that you call home. Give us a call and then we can go over what needs are needing to be met, no matter where you are.    

It Varies

Not every individual served will need lifelong assistance. So when it comes to when do care services end that would be based on:    1) recovery time,
   2) the decision of the one cared for on wanting or not wanting services to continue,
   3) and/or the medical practitioner that oversees your health care says that the goals for recovery needs are met,
   4) and/or goals of maintaining independence with no assistance needed is met.

Planning

With person-centered planning it will be discussed and implemented on what their goals are and our aides will supervise and assist as they work toward the goals they have set.    

A Detailed Definition

Zoom offers a full-featured Basic Plan for free with unlimited video meetings. A Zoom video meeting is where one person hosts and all other participants are invited to attend by link. The proprietor, which would in this case be Family First, will share hosting details with all participants involved. Any participant can share their computer screen. If there are specific people you want involved in your care-making decisions, no matter where they live, they can be a part of these video meetings.

Steps

   1) Go to join.zoom.us.
   2) Enter your meeting ID provided by the host/organizer.
   3) Click Join.
   4) When asked if you want to open zoom.us, click Allow.

What Can You Expect

Once you're in a Zoom meeting, you can use many features such as turning on or off your video and microphone settings, inviting others to the meeting by invite, using the chatbox for communitcating with other participants, recording your meeting, and sharing your own computer screen with the group. I will also document all meetings to be able to have it on file in case you ever want a copy of what was discussed.
